Evaluation of Bone Mineral Density in Patients with Schizophrenia

Abstract:
Schizophrenic patients are exposed to numerous health and physical problems, with low bone mineral density being one of them. Therefore, this study aimed to evaluate this health issue and its relationship with certain biochemical and hormonal variables in patients with schizophrenia. The study was conducted on 56 schizophrenic patients (35 males, 21 females) from the psychiatric department of Zagazig University Hospitals, with a mean age of 38.8 years. All patients were receiving antipsychotic treatment. The Brief Psychiatric Rating Scale (BPRS) and the Positive and Negative Syndrome Scale (PANSS) were administered to the patients. Bone mineral density was assessed using dual-energy X-ray absorptiometry (DEXA), and several biochemical and hormonal variables related to bone density were measured. The study found that 62.5% of the patients had low bone mineral density (osteopenia) in the lumbar spine, and 36.3% had osteopenia in the femoral neck. Furthermore, 19.6% of the patients had osteoporosis in the lumbar spine, and 8.9% had it in the femoral neck. Males were found to be more affected than females. The study also revealed low bone mineral density across all different groups of antipsychotic medications. It was found that vitamin D levels were below normal in 55.4% of the patients. A positive correlation was established between vitamin D levels and bone mineral density among females. The study also found a negative correlation between bone mineral density and the duration of treatment, negative symptoms in patients, mean age, and prolactin hormone levels. The research did not show a relationship between nicotine, body mass index (BMI), and bone mineral density. A statistically significant difference was found between the different antipsychotic drug groups in terms of their effect on bone mineral density. Therefore, we conclude from this research that a large percentage of schizophrenic patients suffer from low bone mineral density, which is affected by various factors in these patients. We recommend routine bone density screening for these patients, along with the necessary biochemical and hormonal measurements. We also recommend conducting further studies on a larger number of patients.
