Abstract:
The hyoid bone (HB) is a U- shaped bone, located anteriorly at the midline of the neck between the thyroid cartilage and the chin. It lies at the level of the 3rd cervical vertebra. It is attached to a large number of well-defined muscles, including infra-hyoid (strap), suprahyoid, pharyngeal constrictor, and extrinsic tongue muscles. Those muscles participate indirectly or directly in the normal functions of the larynx e.g. swallowing and phonation 1. Obstructive sleep apnea (OSA) might occur at different levels: retro-palatal; retro-lingual; or both. The hypo-pharynx is a well-defined site of collapse. The HB is considered an integral part of the hypo-pharynx 2. Riley et al. 1986 developed the hyoidthyroidpexia technique (HTP) to overcome obstruction at the level of the hypopharynx 3. Since that time, the procedure was reported favorable outcomes 3, 4. Rationale: The Hyoid bone (and its attached muscles) could have effects during laryngeal functions as it is considered as a solid part of the hypopharynx. Hyoid bone surgery might affect swallowing and/or voice production. The study evaluates laryngeal functions; namely voice and swallowing outcomes after HTP in patients with OSA. Research Question: Does HTP have any effects on voice and swallowing? Hypothesis: We hypothesize that HTP might affect swallowing and/or voice production. Aim: This study was designed to evaluate laryngeal functions; namely voice and swallowing outcomes after HTP in patients with OSA. Objectives: To evaluate patients with OSA who underwent Hyoidthyroidpexia surgery (after preoperative preparation and the healing period for few weeks). Voice and swallowing evaluations will be done by fiberoptic endoscopy and video-fluroscopic assessment.
