The added value of digital breast tomosynthesis in improving diagnostic performance of BI-RADS categorization of mammographically indeterminate breast lesions

Abstract:
خلفية تظهر النتائج الماموغرافية بشكل أكثر وضوحًا في الصور المقطعية مع التحسن الناتج عن تقارير تصوير الثدي ونظام البيانات (BI-RADS) في تصنيف آفات الثدي غير المحددة. تهدف هذه الدراسة إلى تقييم القيمة المضافة لتصنيع الثدي الرقمي (DBT) لتصنيف BI-RADS في تصنيف آفات الثدي غير المحددة بعد التصوير الشعاعي للثدي الرقمي (DM) كمقاربة أولية. الأساليب والنتائج قمنا بتقييم 296 امرأة مستقبليًا مع آفات الثدي غير المحددة BI-RADS (BI-RADS 0 و 3 و 4) بواسطة DM بين يناير 2018 وأكتوبر 2019. خضع جميع المرضى لـ DBT. قام اثنان من أخصائيي الأشعة بتقييم الآفات وتعيين فئة BI-RADS لكل آفة وفقًا لتصنيف BI-RADS المعجم 2013 باستخدام DM و DBT و DM و DBT مجتمعين. تمت مقارنة النتائج من حيث السمات الإشعاعية الرئيسية والأداء التشخيصي وتصنيف BI-RADS باستخدام علم التشريح المرضي كمعيار مرجعي. تم الكشف عن ما مجموعه 355 آفة على DBT و 318 آفة على DM. تم الكشف عن سبعة وثلاثين آفة بواسطة DBT ولم يراها DM. كانت التشخيصات النهائية لـ 355 آفة 58.3٪ حميدة و 41.7٪ خبيثة. بالمقارنة مع DM ، أنتج DBT ترقية بنسبة 31.5٪ وخفضًا بنسبة 35.2٪ من درجات BI-RADS لآفات الثدي. خفضت DBT عدد BI-RADS 3 و 4 ، مقارنة بـ DM. كانت جميع BI-RADS 4 التي تمت ترقيتها خبيثة. أدى الجمع بين DBT و DM إلى زيادة كبيرة في أداء BI-RADS في تشخيص آفات الثدي غير المحددة مقابل DM أو DBT وحده (P <0.001). خاتمة تؤدي إضافة DBT إلى BI-RADS إلى تحسين أدائها التشخيصي في اكتشاف وتوصيف آفات الثدي غير المحددة للأشعة.
